President Donald Trump hasnt fulfilled his campaign promise to repeal Obamacareoutright, but hes fundamentally reshaped the debate over health care in America in myriad ways during his two years in office.

Along with chipping away at the landmark Affordable Care Act, the Trump administration has made a historic and controversial change to Medicaid, allowing states to require many participants who gained coverage through Obamacare to work or lose their benefits.

How Does It Compare To Biden’s Healthcare Plan

If Biden wins, there will be a different, and seemingly bolder set of priorities for reshaping the ACA.

The possibility of a public option is one of the biggest differences between his plan and Trump’s.

The Biden campaign also says it will eliminate the cap on ACA insurance subsidies, so that people making more than 400% of the federal poverty level qualify for tax credits.

“You’re going to see a big push in trying to make it more affordable,” Emanuel said.

Another campaign promise from the Biden team: lowering the cap on how much coverage in the ACA marketplace can cost, from 9.86% of a person’s income to 8.5%.

Regardless of who wins the election, it’s going to take a lot longer than four more years for this healthcare law to mature.

Just look at how many times Social Security’s been reformed and tweaked, since it was enacted by President Roosevelt in 1935. Or the changes that have been brought to the Medicare and Medicaid systems, since they were signed into law by President Johnson in 1965.

President Obama’s Affordable Care Act has joined this cannon of sweeping reforms. As such, it will take decades to refine, through many more presidents, and many more sessions of Congress. Despite what you might’ve heard about “repeal” or “replace,” improving it will take more continuous, measured improvement of the system than any one President alone can provide.

Will Trump Be Blamed If People Lose Health Coverage

President Trump capped his fruitless four-year journey to abolish and replace the Affordable Care Act by signing an executive order Thursday that aims to enshrine the laws most popular feature while pivoting away from a broader effort to overhaul the nations health insurance system.

Why Did He Do This

For seven years Republicans campaigned against the ACA, which they consider unwarranted government intrusion into a private industry, and for an end to mandated high-priced insurance plans.

However, after gaining control of the presidency, House and Senate at the 2016 election, they could not agree on how to repeal or replace the law.

Killing Obamacare would have left more than 20 million Americans without health insurance, a prospect which sparked uproar. Public approval of alternative Republican health plans hovered at around 20%. And Republicans suffered one embarrassing failure after another when they tried to pass the bills through Congress.

As Trump demanded Obamacares repeal, he also insisted it was imploding. Let Obamacare fail and it will be a lot easier, Trump said after one failed legislative attempt this summer.

The Trump And Republican 2020 Health Care Plan

The Republicans don’t yet have a health care plan less than a year before the 2020 elections.

But based upon their 2017 Obamacare repeal and replace efforts, as well as a major document recently issued by the House Republican Study Committee, what might a Republican plan look like?

First, let’s review the plan House Republicans passed in 2017 during their failed repeal and replace efforts.

The Congressional Budget Office’s July 2017 evaluation calculated the Republican House plan would have spent $1.3 trillion less than Obamacare over ten years on the Medicaid expansion and on insurance subsidies.

The CBO estimated that these funding cuts would have led to:

• Nine million people leaving Medicaid by 2020 growing to 14 million in 2026and then having to enter the individual market as their only option for coverage unless they found their way to employer coverage.
• The individual market shrinking by ten million in 2020 and six million by 2026.
• The CBO therefore concluded that the number of people eligible for the individual market would increase dramatically as the reduced Medicaid population was forced to seek private individual coverage while the individual coverage subsidies were substantially cut.

Obamacare has suffered because a relatively small number of subsidy eligible ever signed up for the programabout 40% at its high point. That in turn has led to very high premiums with an insufficient number of healthy people participating to pay for the costs of the sick.

Oped: Ordered Healthcare Associations But No Follow

This has not happened.Trump issued an executive order on Oct. 12 to ask his Secretary of Labor to propose regulations to allow more employers to make use of “association health plans.” But the actual change has not actually been made yet, noted Timothy Jost, an expert onhealth law as an emeritus professor at Washington and Lee University–so even if millions of people will eventually use these plans, they have, obviously, not been able to do so yet.

Trump added about people joining associations due to changes toObamaCare, “That’s gonna be a big bill, you watch.”

The move toward association health plans is not going to be a bill at all, let alone a “big bill.” This “would be a change in regulation or guidance,” not legislation, Jost noted.
