In 1964, Fact magazine published an unscientific survey asking psychiatrists whether they thought the Republican nominee, Barry Goldwater, was psychologically fit to serve as president of the United States. The problem wasnt that professionals felt the need to share their views of what they considered Goldwaters dangerous ideas it was the irresponsible and often bizarre analyses that were in some cases based entirely on rank speculation. Goldwater is basically a paranoid schizophrenic who resembles Mao Tse-tung, one offered. Another said that he has the same pathological make-up as Hitler, Castro, Stalin and other known schizophrenic leaders. A third said that a megalomaniacal, grandiose omnipotence appears to pervade Mr. Goldwaters personality.

The American public is inadequately educated about mental health. It would take a serious, sustained explication, backed by the power and reach of a professional association, to help us understand why the emotional and psychological stability of our leaders matters and can have an impact on all of us. Every day legal experts weigh in on Donalds unconstitutional or norm-breaking behaviors. Since his covid-19 diagnosis, medical experts have speculated about the course of his illness and the potentially dangerous side effects he may be experiencing as a result of the experimental treatments hes received. Only the mental health experts have been effectively sidelined.

America has been growing more polarized for years. Yet the Trump era seemed to lead to a new peak of polarization. The public now clashes so strongly over the leaders and values of political parties that people may embrace or dismiss a claim based solely on their political affiliation. This concept, known as tribalism, is on the rise, and both liberals and conservatives show this tendency, research suggests. The psychological factors that fuel tribalism include distrusting others based on past experience, avoiding criticism from ones own political party, and protecting ones ego from admitting a mistake or holding a contradictory position.

To any first-year psychiatric resident, Trumps sleepless nights filled with ranting tweets suggest irrational exuberance and lack of control, possibly a sign of a mood disorder called hypomania. His life-long history of disregard for others and deceit, if correct as reported, are characteristic of a personality disorder on the narcissistic and even sociopathic spectrum.

To be sure, definitive psychiatric diagnoses cannot be made without an in-person examination And certainly, psychiatric diagnoses themselves dont make someone unfit to be president. But President Trumps particular history and actions create a high index of suspicion for destructive mental processes which are putting the country and its safety and security in jeopardy.

If we had a functioning Republican Party, this would be the time either to pressure the president to resign early and let Vice President Mike Pence handle the remainder of the term or to invoke the 25th Amendment. At minimum, we would hope that key figures, like Senate Majority Leader Mitch McConnell, House Republican Leader Kevin McCarthy and other senior officials, would act to embrace the reality of the election outcome and put constraints on Trump to stop destructive acts. Instead, they are enabling his worst instincts and behaviors.

Am I the only one who thinks there is method in Donald Trumps madness? Madness it certainly is, on several levels, but in some instances it is purposeful madness.

This madness takes form in President Trumps approach to governing, what can be called the Chaos Theory of Governing. Coming from a background of business where disruptive forces can have a profound effect on the way business is conducted, Trump has adopted a business model to the business of governing. President Trumps chaos theory allows him to play a different, unorthodox game, and as he is the President, others must respond to and play Trumps game. Just what do we mean by chaos theory?

It is often said that chaos is the science of surprises. These surprises are non-linear and therefore, unpredictable. Surprisessurprise us they throw us off. We like the expected, the predictable that to which we are accustomed, the sure thing. Chaos theory would have us expect the unexpected. We dont like that. Ambiguity causes stress. It defies reason. But all is not lost. Perhaps we can achieve some deterministic chaos.

If youve only watched the clips and highlights from Donald Trumps CPAC speech last Saturday, youre not getting the full picture of the explosive horror show that is the worsening status of the presidents mental health. For reasons that defy comprehension, I decided to watch the whole thing live. At the outset, I tweeted that given the Michael Cohen testimony in the immediate rear-view mirror, Trumps CPAC speech was going to be next level crazy. In hindsight, I feel like I low-balled it.

Before we continue, Id like to emphasize that Im not a mental health professional, nor am I an expert in the pharmacological effects of cognitive enhancers like Adderall or Provigil to make a judgment call on the specifics of whats wrong with the president. However, I can say with confidence that somethings extraordinarily wrong with him, and its only getting more dangerous for the nation and by extension the world as time advances.

Remember during the 2016 campaign when Trump mocked Serge Kovaleskis disability? Imagine that for two hours and 20 minutes, from a man who now has the nuclear launch codes in his pants pocket.

Trumps grandiosity and impulsivity have made him a constant subject of speculation among those concerned with his mental health. But after more than a year of talking with doctors and researchers about whether and how the cognitive sciences could offer a lens to explain Trumps behavior, Ive come to believe there should be a role for professional evaluation beyond speculating from afar.

Im not alone. Viewers of Trumps recent speeches have begun noticing minor abnormalities in his movements. In November, he used his free hand to steady a small Fiji bottle as he brought it to his mouth. Onlookers described the movement as awkward and made jokes about hand size. Some called out Trump for doing the exact thing he had mocked Senator Marco Rubio for during the presidential primaryconspicuously drinking water during a speech.
